A Huge Gastric Distention

Question: A 61-year-old man presented to the department of gastroenterology of our hospital after 4 days of vomiting, epigastric pain, and abdominal bloating. He had a history of peptic ulcer disease and atrial fibrillation (AF). His medications were apixaban and esomeprazole. On physical examination, the abdomen was found to be distended but was nontender on palpation. His blood test results were unremarkable. A radiograph (Figure A) and a computed tomography scan (Figure B, C) of his abdomen showed marked gastric distention.
